Connecting with other family carers

Family Carers Ireland offer a new 1-1 support service to family carers. All family carers referred to the service will complete a carer well-being review which helps identify their strengths, needs and the barriers they may be facing in their caring role. A person-centred action plan is developed and agreed in collaboration and individual support is provided over a 12 week period.
Upon receipt of a referral, the family carer will be contacted to complete a short initial assessment before being referred to the correct support manager. It is important that the referral includes the full name, address and contact number for the family carer, as well as permission to share that information with Family Carers Ireland. Any additional information in relation to their caring role and identified needs would also be helpful.
